Any time there is a change in the type of services you are providing and/or if you are providing services to a different regional center, we would love to hear about it!  If you have a change in address/email/fax or phone, we would like to know about that as well.  By updating your membership information, you are assisting us with statistics that are asked for by DDS.  It will also allow us to keep in touch with you.
